Who Does What

QA Manager   Randomly select individuals to receive VOICE reviews.  Mails out “What Adds Value To Your Life” form – to person and Quality Circle Members.  Mails out consent form to person or legal representative.

QA Team              Completes the VOICE Review.

QARC                   Receives any Action Plans generated from a VOICE Review.

QA Manager         Reviews the licensing checklist and determines any items needing to be followed up on during the paper review.   Mails out checklists to licensed providers.

QA Manager        Paper review is separate from the VOICE reviews.

Completes licensing checklist, which also includes.

VA checklist – All programs licensed through DHS.  Psychotropic medication use checklist – As applicable.  Aversive and deprivation procedures checklist – As applicable.  Physical environment and safety checklist .  Verify that all employees and volunteers have completed a satisfactory background study.

QA Manager         Puts together the composite report that includes the findings of all VOICE Reviews, Action Plans, Licensing and VA complaints, and the Paper review report. 

Mails the composite report to the provider and QARC Members.

QARC                  Reviews Composite Reports.  Completes a written recommendation to the County regarding     program licensure.  The recommendation also includes areas of strengths and considerations.

County                Completes the written recommendation to DHS.

¯

DHS                    Reviews county recommendation for licensure.     Issues licenses.
