LICENSING: WHO DOES WHAT?
 
QA Manager Mails out "What Adds Value To Your Life" form - to person and Quality Circle Members.
Mails out consent form to person or legal representative.
 
QA Team Completes the VOICE Workbook.
 
QA Manager Reviews the licensing checklist and determines what items the need to be followed up on during the paper review.
Mails out checklists to licensed providers.
 
Licensing Resource Coordinator
  • Paper review is separate from the VOICE reviews.
  • Completes VA checklist – All programs licensed through DHS.
  • Completes psychotropic medication use checklist – As applicable.
  • Completes aversive and deprivation procedures checklist – As applicable.
  • Completes physical environment and safety checklist (If not already completed)
  • Verify that all employees and volunteers have completed a satisfactory background study.
  • Completes licensing checklist.
  • Assures licensing checklist is complete along with all required checklists.
  • Puts together the composite report that includes the findings of all VOICE reports, licensing complaints, and the paper review report.
  • Mails the composite report to the provider and QARC Members.
 
QA Review Council Completes written recommendation to the County regarding program licensure. The recommendation also includes areas of strengths and considerations.
 
County Completes a written recommendation to DHS.
 
DHS Reviews county recommendation for licensure. Issues licenses.
